Do you have a way of pulling the rod down some and fastening the cap to it and checking the housing bore on that rod? In most cases the rod housing bore is out of spec (egg shaped or lack there of) If this is true then you will have to replace the rod itself. I'd also do my best at trying to mic the crank while its sitting there to get an accurate measurement of the journal. Could be that the journal is not concentric either. Any foreign matter lodged in the oiling system could/can lead to bearing failure. I would certainly get that cleaned out and check the rest of the lubricating system as best as you can.
